Victoria imposes new obligations on Monash IVF after embryo mix-up

The news: The Victorian Department of Health has imposed new obligations on Monash IVF clinics to improve its processes following an incorrect embryo transfer incident at its Clayton laboratory in Melbourne revealed in the middle of this year.

The context: The new conditions include actions and obligations “relating to the process for confirming the intended biological sources for each cycle”, Monash IVF’s patient management system, witness and checking requirements and record keeping, staff education and training, and periodic auditing and reporting to the department.

The company announced in June it had mistakenly transferred a patient’s own embryo back to themself, instead of their partner's embryo. It was the second mix-up it has revealed this year, although the first incident took place in 2023 at a Brisbane clinic.

Monash IVF says it has already implemented safety protocol changes to meet the new process requirements following an internal investigation and an independent review.

Monash IVF does not expect the new conditions to have material impact on existing operations or earnings guidance provided on 20 November.
